You may contact the Clerk's Office on the 2nd floor of the Children's Court if you do no know which agency employs your attorney. The staff can tell you who your attorney is and how to make contact after you show them your identification. Complaint forms are available in the clerk's office.

You should explain your complaint in writing and send it to the agency that the attorney works for.
