What Documents Are Needed To Begin A Case?

Certain documents are required for the initial filing of a civil case; the complaint along with a summons (if issuance is requested) or petition, the Civil Case Cover Sheet, and the Civil Case Cover Sheet Addendum. Examples of common complaint forms are listed in the table below. These are all standard Judicial Council forms except Civil Case Cover Sheet Addendum and Statement of Location (LACIV109), which is a Los Angeles Superior Court local form. The forms are available at the Clerk's Office or on-line. If there is no pre-printed form provided by either the Judicial Council or this Court, the pleading must comply with California Rules of Court starting with rule 2.100.
